    SERVICE LEARNING HOURS OPPORTUNITIES

    Earning student service learning hours is a high school graduation requirement. Students must complete at least 24 service learning hours to meet the graduation requirement.

    It is imperative that you read the section on service learning hours in the most current edition of the High School Graduation Requirements and Four Year Planner published by Prince George's County Public School System.

    Student Service Verification of Participation

    Q: Where do I get the Service Hour Verification Form?

    A: Students can pickup a form from the LHS Guidance Office. Students are reminded that when they turn in the the verification form, that they should have the form signed immediately and keep the yellow copy for proof of completion and submission.
    Or, print out the SSL Verification Form from the PGCPS Service Learning page. NOTE: The printable version does not have a yellow carbon copy attached to it; therefore, it is imperative that you make a copy of the completed form with all signatures for you records BEFORE submitting the form to the LHS Guidance Office.

    Q: Where do I submit the form?

    A: After you have completely filled out the form, obtained all the signatures and made a copy for your records, you will need to submit the form to the LHS Guidance Department.

    PLEASE NOTE: Participating in an activity and not receiving pay DOES NOT always qualify for service learning hours.

    LHS REGISTRATION

    Required Documents
    In order to register as a student at LHS, the parent or legal guardian MUST bring in the following information at the time of registration:

    • Parent's Photo ID
    • Incoming 9th graders:
      • Final report card showing promotion to the 9th grade
      • Incoming 10th, 11th and 12th graders
      • Complete transcript
       
    • Original Birth Certificate (birth certification cards will not be accepted)
    • Immunization records
    • Social Security Card
    • Deed/Tax bill to your home in your name
    • If you renting, you must bring with you:
      • Rental agreement/Lease (Note: The address on the agreement must match the address you are registering).
      • A copy of a cancelled CHECK for your first month's rent/security deposit
      • Two (2) most recent utility bills addressed to you at your current address name
       
    • If you are temporarily residing with someone else, you must have the above listed items along with a notarized statement by BOTH parties indication that you and your child reside at that particular address.


    REGISTRATION CANNOT TAKE PLACE UNLESS YOU HAVE ALL OF THE ABOVE MENTIONED ITEMS WITH YOU!
