POSITION SUMMARY:
The Early
Childhood Case Manger will coordinate, monitor, and implement the “Six Elements
of Case Management” at the Judy Hoyer Family Learning Center Sites located at
Carmody Hills Elementary School, Adelphi Elementary School and Samuel Chase
Elementary School. The Early Childhood Case Manager will work to integrate
service coordination into the day to day operations of the Judy Center. The
integration will include but is not limited to coordinating support to family
for counseling, behavior services for child from birth to age five, and
assistance with housing. The Early Childhood Case Manager will also work with
school based personnel to provide support for children and their families at
the school sites of the Judy Center. The service coordinator will be
responsible for providing families with a foundation of child development
specific to their own child.
DUTIES and RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Ensure
that an individual child and family plan, demographic forms, and other
prescribed forms are completed as needed for referred families utilizing the
Tiered Case Management Model.
Develop
strategies for increasing program visibility and expansion, including
recruitment of families and new family service partners.
Coordinate
case management with Judy Center partners to prevent of duplication of
resources for referred families.
Organize
and maintain a written and electronic database of all services provided and the
key contact person compile and submit required reports to Program Coordinator.
Track
home visits and telephone contacts, maintain written case management reports
for Judy Center evaluation purposes.
Provide
weekly and monthly information and data required for case management reports,
annual continuation grant, and annual Maryland State Department of Education
site visits.
Assist
families in identifying resources and completing documentation to obtain
health, education and social services, as needed.
Coordinate
and plan parent outreach activities for families with assistance from the Judy
Hoyer Family Learning Center team.
Work
with Judy Hoyer Family Learning Center team to generate reports, as needed.
Participate
and coordinate monthly case management meetings with partner programs case
management.
Plan
and implement with the assistance of the Judy Hoyer Family Learning Center team
summer extended learning program.
Attend
Judy Hoyer Family Learning Center Steering Committee meetings as well as
Maryland State Department of Education meetings, as needed.
Visit
classrooms; attend school based meetings to provide assistance for identified
children in from three to five years old who may need support academically,
etc. Assist in providing coordination of services with the school personnel.
Provide
support to work as an integral part of the Judy Hoyer Family Learning Center
Leadership Team to assure efficient delivery of services to families and
children in the respective partnership programs and Prince George’s County
Public Schools.

QUALIFICATIONS:The requirements listed below are representative of the knowledge, skill, and/or ability required.
Excellent
organizational and record keeping skills; Design and maintain electronic
databases.
Assist
in providing follow up services to families and children as needed.
Possess
organizational skills that will support the implementation of the Tiered Case
Management Model.
Possess
the ability to solve complex problems.
Possess
experience in case management and providing direct services to families with
young children.
Demonstrate
the ability to relate positively to colleagues, children, and families to
establish and maintain trusting relationships.
Develop
partnerships with universities and other community partners necessary to
support families;
Demonstrate
the ability to maintain effective working relations with office staff, school
employees, officials, agencies, organizations, and the public.
Preferred
experience in working with families and/or parent training.
Ability
to work independently without supervision and to follow-up and follow through
with tasks.
Ability
to maintain sensitive and confidential records and prepare reports from such
records.
Ability
to work under pressure and make decisions, in accordance with laws, ordinances,
regulations, and established procedures.
Ability
to communicate effectively, both orally and in writing.
Demonstrated
knowledge of early childhood development and appropriate curriculum for
children from birth through age five.
EDUCATION and/or EXPERIENCE REQUIREMENTS:
Bachelor’s degree in Social Work from an accredited
college or university or a closely related area; Master’s preferred.
CERTIFICATION REQUIREMENTS:
Licensed
Bachelor Social Worker – LBSW required; Licensed Graduate Social Worker (LGSA)
preferred.
SUPERVISORY RESPONSIBILITIES:
No
direct supervisory responsibilities, but would supervise interns in connection
with University partners.
