POSITION SUMMARY:
Under
the direction of the Judy Center Instructional Supervisor, the Early Childhood
Case Technician is responsible for all areas related to Judith P. Hoyer Early
Child Care and Education Center (Judy Center) case management, outreach
initiatives, and the maintenance and evaluation of data.
DUTIES and RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Provides
overall case management for children/families; coordinates services, conducts
home visits, attends IEP/SST meetings, maintains telephone logs that document
contact with families, and maintains written case management reports and file
logs;
Performs family outreach by pursuing
families, following up on leads, etc. Advises families of available services
and coordinates services as needed;
Updates, tracks, and maintains the
program database; responsible for the following MSDE evaluation components;
Assist with the identification of
children participating in the meals program;
Implement a Family Case management plan
including family intake, referrals, and intervention;
Coordinates services with partners and
monitor the effectiveness of the various programs;
Tracks client progress and ensure
participation of all partners; maintain daily log of classroom visits and brief
observations;
Provide full day/year round activities
and community/Head Start & Pre-Kindergarten outreach initiatives for children
from birth to age three;
Utilize a database, tracking
children/adults receiving case management services and those who participate in
events for evaluation purposes; provides information to the Program Manager;
Maintain the confidentiality of sensitive
information and prepare reports using such information as needed;
Ensure appropriate coordination between
families interested in enrolling in multiple programs, e.g., child care, Head
Start, Pre-K/Kindergarten, and school;
Assist families with the Child Care
Subsidy (child care voucher) application process and all other available
services;
Assist with planning and participates in
Family Nights, Parenting Seminars, Neighborhood Festivals, etc. For evaluation
purposes, maintain record of families who attend events;
Contact families to remind them of
scheduled events and to encourage participation to increase family involvement;
Attend monthly Steering Committee
meetings, quarterly state-wide Judy Center meetings, and annual leadership
meetings with the Program Manager;
Work closely with Head Start,
Pre-Kindergarten, local behavior specialists, teachers, and families;
Provide the Program Manager with a
bi-weekly case management status update;
Actively participate in the Family
Outreach Subcommittee of the Steering Committee;
Attend hosted or co-sponsored Judy
Center events including periodic evening workshops and events; and
Perform other work-related duties as
assigned.
QUALIFICATIONS:The requirements listed below are representative of the knowledge, skill, and/or ability required.
Ability to work
collaboratively and cooperatively with program staff, school staff, parents,
staff of other community-based agencies, volunteers and community members from
different ethnic, racial, and socioeconomic backgrounds;
Demonstrate ability
promoting teamwork in delivering services to children and families;
Ability to initiate
tasks and work with a team and independently with minimal supervision;
Excellent
organizational and record keeping skills; design and maintain electronic data
base;
Demonstrate a
high level of human relations, oral, written, communication, planning and
organizational skills;
Assist in
providing follow up services to families and children as needed;
Possess
experience in case management and providing direct services to families with
young children;
Ability to
maintain sensitive and confidential records and prepare reports from such
records;
Thorough
knowledge of McKinney-Vento Homeless Assistance Act, as amended by the No-Child
Left Behind Act of 2001;
Ability to
effectively work with principals, specialists and teachers to implement and
monitor the program in schools;
Ability to
work effectively with Homeless Education Office staff;
Ability to
relate effectively to and work with parents, students, staff and external
agency representatives; and
Ability to
work independently and to exercise initiative in carrying out program duties
and responsibilities.
EDUCATION and/or EXPERIENCE REQUIREMENTS:
Bachelor’s
degree from an accredited college or university in Early Childhood Education,
Social Work, Human Resources or a related field preferred; Graduation from High
School or a GED required, with at least five (5) years’ experience with
families in a social services setting; or any equivalent combination of
experience and training which provides the required knowledge, skills and
abilities necessary to perform effectively in the position.

SUPERVISORY RESPONSIBILITIES:
No direct supervisory responsibilities.
