POSITION SUMMARY:


Under the direction of the Instructional Supervisor, the Home and Hospital Case Manager is responsible for the coordination and facilitation of the home instructional program in both face-to-face and virtual learning modalities for the district.  The Case Manager ensures educational services are provided for each qualifying student for the time-period designed by the student’s physician or mental health professional or for students serving long-term or extended suspension.

DUTIES and RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Assists with the coordination of instruction with other offices (school counseling, health services, 504 and psychological services) to ensure students are processed and served according to COMAR;

Processes incoming referrals and assigns a teacher to qualified students within 10 school days of receipt of referral documents as specified in COMAR;

Gathers information needed for administrative decision making regarding student approvals;

Assists with recruitment, interviewing and selection of new part-time home and hospital teachers and maintains a database with eligible candidates;

Assists in the preparation and analysis of data reports;

Consults with the director, supervisor and other staff members to establish program standards, develop office goals, develop a data monitoring system and ensure effective program delivery;

Maintains a database on student referrals to include fields for approvals, denials, start and end of instruction, assigned teacher, etc.

Oversees the daily operation of the office, staff-programming, payroll, student/home teacher assignments, and home and hospital teacher needs;

Manages temporary hourly paid staff who render face-to-face instruction for home and hospital services for alternative academic supports, as well as those serving the Home Teaching Library;

Trains, mentors and monitors new home and hospital teachers regarding procedures and delivery of instruction;

Maintains the administrative procedures, forms and pertinent programmatic documents;

Provides written and oral communication related to verification, approvals, denials and service requirements;

Facilitates training school staff members, community organizations and agencies, area hospitals and parents/guardians regarding protocols and procedures for face-to-face or virtual home instruction;

Serves as a consultant and partner by attending school-based meetings (SIT, SST, IEP, 504) as well as CIEP, compliance, court liaison and inter-agency;

Represents the supervisor and/or department at Maryland State Department of Education (MSDE), area hospitals, county agencies and other community providers; and

Performs other duties as assigned.

QUALIFICATIONS:The requirements listed below are representative of the knowledge, skill, and/or ability required.

Knowledge of federal and state laws and regulations relevant to HHT (e.g. Individuals with Disabilities Education Act; Section 504; MSDE Regulation COMAR 13A.10.01.01) and PGCPS Administrative Procedures regarding HHT;

Knowledge of school organization, goals and objectives, curriculum, teaching techniques, current trends, research and materials of instruction in field of specialization;

Excellent knowledge and understanding of the teaching and learning process;

Considerable knowledge of methods and strategies in one or more of the following areas: elementary school, middle school, high school, special education, early childhood, multiple handicapped and severely profoundly handicapped;

Excellent time management, problem solving, conflict resolution and organizational skills;

Ability to work effectively with students, parents, administrators, colleagues, community and other school system staff;

Flexibility, creativity, ability to adapt to a variety of teaching settings;

Ability to communicate clearly and effectively, both orally and in writing and to maintain effective working relationships; and

Skill in using the computer and other technological applications.

EDUCATION and/or EXPERIENCE REQUIREMENTS:


Bachelor’s degree required in applicable field of education from a regionally accredited college or university (Master’s degree preferred); and five (5) years of successful teacher experience (Teacher leadership experience preferred). 



CERTIFICATION REQUIREMENTS:


Holds or is eligible for a Professional Certificate appropriate to area of assignment issued by the Maryland State Department of Education.

SUPERVISORY RESPONSIBILITIES:


No direct supervisory responsibilities.
